Code on Dental Procedures and nomenclature

Code Change Request Forms

There are three Code Change Request Forms available, one each for the specific actions noted below. These forms are Microsoft Word © documents that are intended to be downloaded to your computer, completed at your convenience and returned, preferably by e-mail, to dentalcode@ada.org.

After reviewing the Guidelines, Evaluation Criteria and Instructions, please complete and return the applicable change request form (plus any attachments) as noted in the Instructions page.

Please answer all the questions that are on each type of change request:

Please include additional sheets as necessary where written answers exceed the space available on the form. When doing so please include the number of the question being addressed.

The change request forms includes questions that concern the various components of a dental procedure code (e.g., Nomenclature, Descriptor). Please review the Components of A Dental Procedure Code for an explanation of each of the three parts.
