you have been denied insurance or issued insurance on a non-standard
basis, this may have been reported to the MIB. If you apply
to another MIB member insurance company, it may obtain the
information the MIB has about you in its files. Upon receipt
of a request from you, the MIB will arrange disclosure of
any information it may have in your file. If you question
the accuracy of information in the MIB's files, you may seek
correction in accordance with the Federal Fair Credit Reporting
Act. Upon payment of an $8.50 fee you can Request a Copy
of Your MIB Record.
