ADA Members: Fill Out Profile Questionnaire
Each month, we will feature a profile of an ADA member in the ADA.org Member Center. If you would like to be considered for this, please fill out the profile questionnaire (please note that this link will open in a separate window so you may need to disable any pop-up blockers). There are many questions to choose from and it may take you 10-15 minutes to complete. The more questions you choose to answer, the better chance that your profile will be shared on ADA.org.
If your profile is selected to be featured on ADA.org, we will contact you prior to the posting for your final approval. We will select members to profile on a first-come basis, and also consider diversity in both geographic location and responses to the questions.
