ADA Foundation: Grants/ Scholarships: Scholarship Programs

2009 Update

  • Please note:  for the 2009-2010 cycle, dental school officials should submit one application for each scholarship program (i.e.,  the Underrepresented Minority Dental Student Scholarship Program and the Dental Student Scholarship Program)
  • Fewer scholarships will be available for the 2009-2010 cycle. The Underrepresented Minority Dental Student Scholarship Program will award at least 10 scholarships.  The Dental Student Scholarship Program will also award at least 10 scholarships.  All dental student scholarship awards remain at the $2,500 level.

Application Process

  • Only schools accredited by the Commission on Dental Accreditation of the American Dental Association are eligible to participate in the ADA Foundation’s Scholarship Program.
  • School officials please watch for an e-mail from Rose Famularo at the ADAF, to which application forms will be attached.  School officials may contact the ADA Foundation by phone at 312-440-2763 or by e-mail: famularor@ada.org to request the scholarship application forms.
  • Students must contact the associate dean for student affairs at the school to request a scholarship application form  because  the forms are exclusively disbursed by school officials.
  • For 2009, for each scholarship program, the appropriate dental school officials should select a top student.  The ADA Foundation is accepting one application from each school for each scholarship program to consider.

Applicants must submit to their schools:

  • Application forms must be typed and be signed by school officials;
  • A completed application form includes the Academic Achievement Record Form and the Financial Needs Assessment Form, signed by the school official;
  • Two completed reference forms, sealed and signed on the back flap of the envelopes by the referrers (required forms must be used which are a part of the scholarship application form);
  • Typed, biographical sketch questionnaire (required form must be used which is a part of the scholarship application form);]
  • A completed W-9 Form must be included as it is a part of the scholarship application form, and signed and dated by the student;
  • If the applicant or school requires confirmation of receipt of the application, please arrange that confirmation with the shipping vendor or U.S. Post Office.  The Foundation will not be able to verify over the phone or by e-mail.

Schools should submit their one completed application for each scholarship program to the ADA Foundation by the October 19, 2009 deadline date for consideration.

The following scholarships will be offered for the 2009-2010 academic  year; however,  based on available funding:

Dental Student

Underrepresented Minority Dental Student

Submission Deadline Date

All scholarship applications and supporting documents for all scholarship programs must be received in the ADA Foundation office by October 19, 2009.  If an application is received after that date, it will not be considered.

Notification

Notification of funding decisions will be made after December 31.

Please Note:  Only students who are awarded scholarships will along with their school officials receive follow-up communication from the ADA Foundation.

Criteria for Each Scholarship Program

Dental Student Scholarship

The maximum annual award for the Dental Student Scholarship is $2,500.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
  • Applicants must be an entering second year student at the time of application and currently attending or enrolled at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate a minimum financial need of $2,500;
  • Applicants must have a minimum accumulative grade point average of 3.0 based on a 4.0 scale;
  • Two reference forms from two dental school representatives (i.e., professor or academic advisor) in support of the application must be submitted as part of the application form.

Underrepresented Minority Dental Student Scholarship
The maximum annual award for the Underrepresented Minority Dental Student Scholarship is $2,500.

The Underrepresented Minority Dental Student Scholarship Program mirrors that of the Dental Student Scholarship. Certain minority groups have been identified as being underrepresented in dental school enrollment. At this time, African American, Hispanic and Native American students are eligible to apply for this scholarship.

The ADA Foundation’s Underrepresented Minority Dental Student Scholarship Program is funded in part by Harry J. Bosworth Company, Colgate-Palmolive, Proctor & Gamble and Sunstar Americas.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
