Members and nonmembers of the American Dental Association may submit written testimony for reference committee hearings by downloading and completing the written testimony form. This form must be completed in full and sent to the appropriate Reference Committee email address below to be processed. Written testimony is limited to 300 words. Any testimony that is submitted with more than 300 words will be returned to the author with the request to reduce the word count.
To be considered by the reference committee, comments must be received no later than 5:00 p.m. (Central Time) on Monday, October 5, 2026. Written testimony received by this deadline will be available for viewing by reference committee members and other members of the House of Delegates, and those with access to the House of Delegates (such as the ADA Executive Director, Council and Commission chairs, executive directors of state dental associations, and ADA staff). The contents of the written testimony forms will be posted in their entirety, including names, affiliations as well as any personally identifiable or confidential business information included on the form.
Please keep comments brief and consistent with the ADA Principles of Ethics and Code of Professional Conduct.

Delegates and members who will be in attendance at the reference committee hearings and wish to provide testimony will be allotted two minutes of testimony per resolution. To ensure fairness and consistency, written testimony should not be used to supplement or circumvent the two-minute oral testimony limit that applies equally to all individuals providing testimony at reference committee hearings.
