Registration Options
- Register online (credit card only).
- Register by mail: Complete downloadable form, print and mail to: ADA, New Dentist Committee, 211 E. Chicago Ave., 20th Floor, Chicago, IL 60611-2678. Must be postmarked by July 1, 2013.
Please print or type all information. A check payable to the ADA or credit card authorization for registration fees must be received by the ADA at the time of registration. Registration forms without payment will not be processed.

Early-Bird Registration Sweepstakes
- Enter by May 24, 2013 for a chance to win one of the following*:
- One complimentary registration to the 2013 New Dentist Conference, July 18 – 20, 2013 (estimated retail value $275 – $525).
- Three nights lodging at the Four Seasons Hotel, July 17 – 20, 2013 only (estimated retail value $550)
- Two complimentary registrations to ADA Annual Session in New Orleans, Oct. 31 – Nov. 3, 2013 (estimated retail value $75 – $100).
- Three nights lodging at the Sheraton New Orleans, Oct. 31 – Nov. 2, 2013 only (estimated retail value $700).

Registration Deadlines
Early Bird Sweepstakes Registration Deadline:

Hotel Reservations Deadline: June 14, 2013
Pre-Conference Registration:
Register online by Noon, CDT, July 8, 2013 or send in the mail, postmarked by July 1, 2013.
On-Site Registration Hours
Thursday, July 18, 2013: 7:30 – 5:30 p.m.
Friday, July 19, 2013: 7:30 – 4:30 p.m.
Please register early as courses fill quickly. Attendees will be registered for courses on a first-come, first-served basis. After July 8, 2013, you may register on-site at the Four Seasons Hotel (an additional $50 registration fee will be applied). Check, credit card and money order payments will be accepted — cash payments will not be accepted on-site.

Guests of registered attendees are welcome to attend both the Opening Reception and the Friday Night Event at Coors Field. The Opening Reception fee for adult guests (13 years of age & older) is $15 per person. Children 12 years of age and under are free. The Friday Night Event fee for guests of all ages is $75 per person.
Cancellation Policy
All refund requests must be received in writing at the ADA New Dentist Committee office no later than July 8, 2013. Approved refunds or credits will be issued following the conference. Requests received after July 8, 2013 will not be eligible for a refund.
